We assist older adults with their total care needs by connecting them with resources unique to their situations.  After a complete and thorough needs assessment, we work with the families to develop a personalized service plan that will allow loved ones to live with the greatest degree of independence, safety, and comfort.  For older adults in need of greater care support, we assist caregivers in making the best relocation decisions.

Care Management is an important source of assurance for family members overwhelmed by caregiving responsibilities whether local or from a distance.  We are your in-town professional, or you may think of us as your in-town daughter!

• Conduct a comprehensive in-person assessment to identify your current situation and care or support needs.
• Develop a customized service plan with care options.
• Educate and support caregivers and seniors so they can make well-informed choices.
• Provide community resource information and referrals. 
• Screen, arrange, and monitor necessary in-home services identified in the service plan.
• Follow-up and monitor care services in home or in a senior care facility.
• Reevaluate the service plan regularly and make necessary changes.
• Assist with long-distance caregiving.
• Identify environment safety concerns and provide remedy options.
• Oversee basic home modifications to assure safety.
• Accompany clients to doctors' appointments and provide necessary information and support.
• Assist with moving an older adult to senior housing community, assisted living, or a nursing home.  
• Help you choose the living arrangement appropriate for your situation. We do the legwork for you by connecting you to organization that screen the facilities for appropriateness and make the initial contact with facilities to check availability and accompany you on facility visits.
• Provide a local presence with regular visits to assure quality of care and services provided by homecare companies in the home or in a senior community/facility.  We are your advocate!
• Establish a personal and trusting relationship with seniors and their families.
• Assist with daily living needs.
• Communicate regularly with caregiver updates and alert families to problems and status changes.
